The Enhance Programme was launched in end of April by Leeds Older People’s Forum and Forum Central. At ABA, we are overwhelmed to be one of 14 delivery partners in the city to develop and deliver ways to support South Asians and BAME elderly people in their own homes. We will be providing a city-wide service working in partnership with the Neighbourhood Teams NHS, Leeds Community Healthcare NHS Trust, social care partners and Hamara Healthy Living Centre to create better-personalised care and support for individuals, including in relation to safe discharge home from the hospital.
