Enhance Project.

Enhance is a citywide service to support safe and sustainable discharge from hospital and neighbourhood teams to a secure home environment. This service will enhance capacity in both sectors and avoid both delayed discharges and readmissions.

Who is Eligible?

Through ABA we are currently accepting referrals from anyone:

  • aged 18 and above
  • from a Black, Asian or Minority Ethnic community
  • Who is leaving hospital for any reason and are in need of support services

 

Although the majority of referrals will be from the Neighbourhood Teams, the Enhance programme can accept relevant referrals from other community services and self referrals.

What Support Could You Get?

We offer a range of support as a part of Enhance to support our service users. 

  • Making meals or refreshments and enabling the person to make food for themselves
  • Ensuring the service users have enough provisions, topping up pre-payment meters and ensuring utility bills have been paid
  • Prompting the service user to take their medication
  • Undertaking joint visits with healthcare professionals and neighbourhood teams to work together on how the patients’ goals and needs will be achieved
  • Prompting the service user to undertake exercise and social activities
  • Supporting the service user with self-management. Facilitating conversations that focus on their goals, motivate and encourage
  • Provide regular contact, befriending  and building a positive relationship to  improve confidence and independence
  • Support the person to seek help to resolve any issues or concerns they have
  • Linking them with community activities and social connections
  • Assisting with IT
  • Ensuring adequate equipment is in place to enable safe movement around the house and carrying out a falls prevention assessment
  • Referring patients with dementia to a dedicated dementia support worker

 

This list is not exhaustive and we will aim to provide support in any way we can.
